As a client or client representative, you have the right to:

- Request restrictions: Limit how we use or disclose your PHI
- Receive confidential communications: Choose how we contact you about health matters
- Request an accounting: See a list of certain disclosures we’ve made of your PHI
- File a complaint: Report concerns if you believe your rights have been violated
Requests to exercise any of these rights must be submitted in writing.
Safeguards We Have in Place
We use a combination of administrative, physical, and technical safeguards to protect PHI, including:
